Pick a question with an unmistakable answer

Use a short fixture containing an exact, uncommon phrase and a simple fact. Ask for that fact before testing broad summaries. A general question makes it harder to tell whether the assistant used the document or background knowledge.

Write down the expected answer and where it appears. Confirm that you selected the intended document or collection for the current conversation.

Follow the evidence in order

  1. Extraction: Does the stored text contain the known sentence?
  2. Processing: Did indexing complete, with no failed embedding jobs?
  3. Selection: Is this conversation using the expected source and permissions?
  4. Retrieval: Does the known passage appear among retrieved results?
  5. Assembly: Does the final model input include that passage?
  6. Answer: Does the model interpret it correctly?

Stop at the first missing artifact. Changing later stages while an earlier one is broken adds noise.

Repair the specific stage

If extraction is empty, choose an appropriate parser or OCR workflow. If indexing failed, inspect the embedding service, model identity and collection configuration. If selection is wrong, correct the attached source or access policy.

If the passage exists but retrieval misses it, compare an exact-term query with a paraphrase. Review chunk boundaries and filters before increasing the result count. Returning more irrelevant text can make the final answer worse.

If the correct passage enters the prompt but the answer remains wrong, simplify the question and require explicit evidence. That is a generation problem, not an upload problem.

Confirm both positive and negative behavior

The fixture question should now return the known fact with its source. A second question whose answer is absent should remain unanswered or explicitly uncertain.

Keep both cases. Fixing a missing answer by encouraging the model to guess creates a different failure.

Preserve the trace

Save the document identifier, parser and embedding versions, selected collection, retrieved passage and output. Redact private contents before sharing logs.
